Instructions is given in 1 post - after you donated sent pm to LMS (on forum) and spesify you exact main character name and amount donated.

i have donated today and sent PM already but no rewards yet. I will just wait but I'm excited already.

As described in first post, reward could take upto 24 hrs, but usualy its from less then hour to half day (depend on difference in timezones)
